Hilary C Siebens MD, Principal, Siebens Patient Care Communications LLC, contributes to initiatives focused on achieving whole person/patient care. Her work is informed by experiences in PM&R and geriatric primary care. Patients have included healthy older adults, those with multiple chronic conditions, and those requiring palliative and hospice care.
Clinical research has included exercise for hospitalized seniors, stroke and hip fracture rehabilitation studies, computerized adaptive testing for functional outcomes, and a recent RCT of nurse-led care management in Parkinson’s that demonstrated improved care quality. She has been a first author and co-author on these studies’ publications and has participated on CARF, AHCPR, and NIH expert consensus panels.
Evidence-based tools to help in clinical communications have emerged from this research: (1) a whole person 4-domain organizing framework (Siebens Model of Clinical Domain Management) with applications in team conferences, care management, and documentation, and (2) a self-care Notebook for Wellbeing & Health Care for individuals to keep their health information in one place and to help communicate among multiple providers. Her consulting focuses on integrating tools like these into current day practice in efficient user-friendly ways that capitalize on nursing and provider expertise. Clients have been in private and public sectors.
She has practiced at Cedars-Sinai Medical Center, Spaulding Rehabilitation Hospital, Massachusetts General Hospital, University of California-Irvine, and the Long Beach and Greater Los Angeles Veterans Affairs Medical Centers. She has been a founding medical director of two inpatient units and an outpatient primary care senior center.
She received her MD at Harvard Medical School and clinical training, with Board certification, in internal medicine at Johns Hopkins, PM&R residency at Tufts New England Medical Center, and geriatrics fellowship at Harvard. She has had academic appointments at UCLA, Harvard, UCI, and University of Virginia.
She appreciates vegetable gardening, visiting national parks, swimming in efforts to stay physically fit, riding a 50 year-old 3-speed rusty English bicycle, and just plain hanging out with her husband, children and spouses, and grandchildren in southern California.
