General PMR patients are becoming more complex. The Cardiopulmonary Rehabilitation models of care are an effective Systems Based approach to treat and prevent future complications including: progressive debility, declining independence, and hospital readmission - outcomes relevant to all Pysiatrists and health systems.
This model will be illustrated through discussion and exploration of the key role of Physiatry in supporting:
• Advanced Heart Failure including mechanical support, transplant and ECMO
• Pulmonary Transplant
• Effective transitions of care from Acute care - Rehabilitation - Outpatient and Virtual treatment at home
• Implementation/Utilization of Value Based Care models, data collection and analysis to improve Health System outcomes including increased throughput, decreased length of stay, and reduced hospital readmission
Physiatrists are coming into clinical practice with more medical knowledge than ever before. The steadily increasing acuity of general rehabilitation patients in the in-patient setting translates into a robust skill set enabling the modern physiatrist to interface and contribute to medical teams as they provide specialized care to complex populations. All physiatrists can contribute to these teams a theme that will be developed in this session.

Learning Objectives:

  • Summarize current state of Cardiac Rehabilitation interventions for advanced heart failure including: mechanical support, transplant and ECMO.
  • Summarize current state of Pulmonary Rehabilitation interventions for advance lung disease including pulmonary transplant.
  • Illustrate utilization of Value Based Care models in special populations including data collection and analysis improving Health System outcomes including increased throughput, decreased length of stay and reduced hospital readmission.
  • Discuss current transitions of care for patients with medically complex rehabilitation needs and identify participant opportunities to implement and coordinate optimal transitions of care as part of ongoing Physiatric care.
