Azlan Tariq D.O. FAAPMR CIME is a Board Certified Physical Medicine and Rehabilitation physician and the Chief Clinical Officer for Integrated Rehab Consultants, a nationwide group of physiatrists practicing post-acute rehabilitation.
He earned his undergraduate degree at Stony Brook University and received his medical degree from Lake Erie College of Osteopathic Medicine. He completed his Physical Medicine and Rehabilitation residency at Marianjoy Rehabilitation Hospital in Wheaton, Illinois during which he was awarded the Resident of the Year honor and was involved in various committees and leadership roles. He went on to complete additional fellowship training in Pain and Musculoskeletal Medicine at University of Wisconsin-Madison.
He has lectured at various conferences around the country and he is the current chair of the AAPM&R SNF member community.
Dr. Tariq and his PA’s currently provide PM&R services to various Post-Acute facilities in the Chicagoland area. Dr. Tariq is also the owner and founder of Optimal Health Medical Fitness and Illinois Institute of Regenerative Medicine. His outpatient practices are dedicated to providing the most advanced non-surgical orthopedic and regenerative medicine treatments in the Chicagoland area.
