Session Description: Health care fragmentation is recognized globally. Its consequences are poor standards of care, patient dissatisfaction, unsustainable health care costs and inferior quality of care.
In reducing fragmentation and fostering integration, one can only hope for better care and mitigating unintended consequences.
Care Coordination is critical for obtaining optimal results in patient management.
The session will address Health care Fragmentation and provide practical tips for Care Management and strategies for Integrating Episodic Rehabilitation Care with Longitudinal Care.

Learning Objectives:

After attending the session, the participants understand roles of proactive assessments of individuals with enduring (chronic) health conditions.
At the conclusion of the presentation the attendees will gain an appreciation of warm versus “cold” hand-off.
At the conclusion of the presentation the attendees will be able to describe four domains to assess and guide whole person care for episodic and longitudinal care.
As a result of attending this session, the attendees will be able to consider roles of hard copy print medium in individual’s own self-care.
At the conclusion of the session, the attendees will appreciate person/patient-centered care requiring care coordination between episodic and longitudinal care providers.
At the conclusion of the session, the attendees will be able to review barriers and facilitators of communication between episode of care and longitudinal care providers.
The attendees will be able to recognize the effects that technological innovations such as remote patient monitoring and remote therapy monitoring will have on care coordination moving forward.
