Session Description: This session is designed to review cases that are challenging yet common in any outpatient spine and musculoskeletal physiatry practice. We will discuss each case from top to bottom, reviewing the clinical scenario, relevant imaging, interventions and the challenges experienced throughout the entire process of assessment and treatment. Our expert panel will opine on their strategies to address the challenges. We aim to make this interactive with audience input as well.

Why is this session needed? : The session's goal is to review cases that are challenging in terms of both diagnosis and treatment and utilize an expert panel to discuss evidence based strategies to overcome the clinical hurdles. We will present 4-5 specific cases we have encountered in our clinic, review the clinical presentation, relevant imaging, interventions performed and dissect the challenges in each of those sections. The panel members will debate on best practices to address the difficulties of each part of the case, including the potential diagnoses, imaging findings, procedural difficulties. We'd like to make this interactive experience and poll the audience members on how they would handle the challenges of each case.

We believe this session will provide high quality education to attendees that are in training and also the more seasoned attendings and will provide a venue to discuss challenges we all face in our clinical practice. It will be an excellent opportunity to review literature that affects our practice and clinical decision making that typically happens real time (e.g. during a procedure).

Examples of cases will include the following topics: physical exam and imaging correlation, procedural decision making (for example, interlaminar versus transforaminal epidural), procedural imaging and flow pattern analysis

As a result of attending this session, participant will be able to improve in procedural decision making based on case presentation and imaging.
As a result of attending this session, participant will be able to improve diagnostic strategy in difficult clinical scenarios involving spine mimickers.
As a result of attending this session, participant will be able to interpret suboptimal fluoroscopic flow patterns and strategize on alternative procedures for the same diagnosis.
As a result of attending this session, participant will be able to share their input on challenging yet common cases.
